Request Duo Token

Fillable form to request token for Duo to be used in place of a personal cell phone.

It is recommended that you use auto-push along with the Duo app on a mobile phone. If your phone does not have the capability of using Duo or sending a text push, you will need a token to authenticate.

Note: First token request is free of charge. Additional or replacement tokens are $10 each.
